Incidents

Record and investigate all accidents, near-misses, and adverse events at the club. The module supports RIDDOR-reportable incidents and corrective action planning. All incidents are timestamped, categorised by type and location, and linked to the responsible department.

Common actions

    Report an incident: tap "+ Report Incident" and fill in the incident type, location, date and time, persons involved, and a description of what happened. All fields are timestamped on submission.

    Flag as RIDDOR-reportable: if the incident meets RIDDOR criteria (specified injury, over-7-day incapacitation, dangerous occurrence, or work-related disease), tick the RIDDOR checkbox. The record will appear in the RIDDOR log for your statutory reporting records.

    Add corrective actions: in the incident record, use the Corrective Actions section to note what steps are being taken to prevent recurrence. Include who is responsible and a target completion date.

    Investigate and close: once the investigation is complete, update the outcome notes and change the status to Closed. Closed incidents remain on record and are included in trend reports.

    Filter by type or status: use the Type filter (Accident / Near-Miss / Dangerous Occurrence / Other) and Status filter (Open / Under Investigation / Closed) to find specific records.

    Review trends: use the AI Assistant to identify patterns โ€” ask "How many incidents happened in the last 6 months?" or "Which area has the most incidents?" to spot recurring hazards.

Record near-misses โ€” not just injuries. A culture of near-miss reporting is a strong indicator of good safety management. H&S inspectors look favourably on clubs that show they are proactively identifying and addressing hazards before someone is hurt.
